is it a new chain? i know new chains will tend to stretch a bit which will cause the loose look. also did the bolts actually back out or the chain was just loose?

no, the chain is probably the original. (2001 owner never rode it) has 12t front and stock rear. It just came loose, all of the bolts were still tight.

how tight did you tighten the chain? if too tight it will stretch. what kind of chain is it and how old? cheap chains do not usually last very long.

you may have to resort to buying a chain sprocket combo and start from scratch.
